What documents must be completed at the time of RCFE admission?

At admission, CDSS requires the execution of a signed admission agreement (LIC 625), a pre-admission appraisal (LIC 601 or equivalent), a physician report confirming the resident's capacity for residential care (LIC 602), and a resident rights acknowledgment. The care plan must be initiated and completed within 30 days.

What is the pre-admission appraisal?

The pre-admission appraisal is an assessment conducted before admission to determine whether the resident's needs can be met in an RCFE. It covers functional abilities, health conditions, behavioral history, medication needs, and special care requirements. CCR §87457 requires this assessment before or at the time of admission.

When must the initial care plan be completed?

CCR §87468 requires that a care plan be developed for each resident. Best practice and CDSS expectation is that an initial care plan is in place within 30 days of admission. The checklist includes a 30-day milestone review to ensure the plan is initiated, documented, and signed by the appropriate parties.

What is a MAR and when must it be set up?

A Medication Administration Record (MAR) or Medication Assistance Record documents every medication the resident takes, who assisted, the dose, and the date and time. For residents receiving medication assistance, the MAR must be in place before the first dose is given. This checklist walks through the MAR setup steps immediately following the admission agreement signing.
