Under CCR §87457, an RCFE must conduct a written assessment of a prospective resident's functional status, medical needs, cognitive abilities, and behavioral characteristics before admission. The appraisal determines whether the facility can safely meet the resident's needs.
The appraisal is the assessment (gathering information about the resident). The needs and services plan is the document that translates the appraisal findings into specific care commitments: what services the facility will provide and how.
The initial care plan is due within 30 days of admission. Subsequent reviews are required every 6 months and annually. The plan must also be updated whenever the resident's condition changes significantly, after any fall or health event, or at the request of the resident or responsible party.
For residents with medical complexity (including bedridden residents, G-tube users, or those with significant diagnoses), physician involvement and often a signature is required. The document includes physician signature blocks for these situations.
Yes. Under CCR §87463, residents and their designated responsible parties have the right to participate in the development and revision of the needs and services plan. The document includes fields to document resident and family participation and consent.
